In order to maintain an effective flow of information and product shipments for repair or service, you may want to have the following information ready to help complete the RMA form you are about to fill out. Some of this information is not essential, but may be helpful in completing the process more quickly.
Please have this information ready
- Part number(s) of the failed item (required)
- Serial number(s) of the failed item (required)
- Original purchase order number (optional)
- Open Repair Purchase Order (optional)
- Any shipping instructions, carriers, carrier account numbers, etc...(optional)
- Any Own reference numbers (optional)
